The person who claimed it fell off never proved it, and have never heard from them since. Nor is there any other reports of it falling off.

It is easy... ish. The bit with the pliers is well easy, but advisable you do it to avoid the constant slippage frustration...
Other than that, it's more of a time-consuming easy than anything else (and that's mainly if, like me, you're learning how to do it on the fly)

Acra, i've done the plier thing. Its definatly made it more secure, but haven't tried to mount it yet. hopefully the brass hasn't gone too down hill! lol.

Mine did that - what you wanna do, is this:
take the chrome thing off
get a wide set of pliers
bend the tops of the lugs over the ring
fit back on pipe
tighten to fullest (carefully - it's only a brass screw!)

That should stop the slippage!
